What should surprise the reader: A failed cardiovascular outcome trial is not the same as a zero-value company. The balance sheet can fund time, while the same balance sheet and operating losses can make the lost royalty difficult to replace. The trade's immediate problem is not a lack of bearish evidence. The base case clears the favorable 10% screen, and the probability-weighted price-only result is positive for a short. That is not enough. The adverse path is more than four times the permitted 5% boundary, and the gross ratio is below 2:1 before the borrow fee, spread, slippage, funding, recall, buy-in, and event-gap costs. What Should Surprise the Reader

  1. The failed trial does not directly remove all Ionis revenue. The June 10-Q shows $226.4 million of first-half commercial revenue, but most of that figure is royalty revenue and the company remains loss-making at the operating level.
  2. The cash balance is a floor against immediate financing stress, not a valuation floor. At June 30, Ionis held $2.0546 billion in cash and short-term investments against $1.3226 billion of convertible notes, a $562.6 million royalty-sale liability, and $262.5 million of long-term lease liabilities.
  3. The royalty-sale liability matters. Ionis received $500 million upfront for a minority interest in future SPINRAZA and pelacarsen royalties. Those facts prevent a responsible analyst from treating the program as legally or economically extinguished. They do establish that the pivotal outcome evidence failed the endpoint the market needed for a straightforward approval case.

The balance sheet shows why the short is a time-value argument rather than a solvency call. Ionis had $350.1 million of cash and $1.7045 billion of short-term investments at June 30. It owed $770 million of 0% convertible notes due 2030 and $600 million of 1.75% convertible notes due 2028 at principal value. It also carries the royalty-sale liability and long-term leases. Six-month operating expenses were $733.8 million and the operating loss was $219.7 million. The company says its cash, investments, and plans for future cash are sufficient for planned operations, but it also warns that failure to commercialize medicines or license assets could require additional funding.

The core commercial base is growing but small relative to the spend. TRYNGOLZA sales were $31.7 million in the first half and DAWNZERA sales were $42.4 million.
